License Renewal and CME FAQ
CME FAQ: What is Category 1-A and Category 1? |

License Renewal Application Blank paper form for 2012-2013 renewal cycle for licenses with 12/31/2011 Renew By date. This form is a fillable PDF; however, the renewing licensee must sign and date it before submitting it by mail, email, or fax. Please be aware that this is not the Online Renewal Form, and submitting this form means it will take up to 30 days for you to receive your wallet card. If you need your application expedited, please use the Online Renewal System. |

CME Audit / Extension Reporting Form If your license was randomly selected for CME audit, or you are requesting an extension, you must report your completed CME activities for this renewal cycle. When you have completed your 40 total CMEs, type the information in this form on your computer, print and sign, and email, mail, or fax with copies of Certificates of Completion or your AOA transcript showing completion of a 40 CME total. |

Request for Extension of Time to Comply With CME Requirement By filing this request for extension, you are automatically included in the CME Audit. Carefully read the requirements listed on this form for completing the renewal of your license when filing an extension. This form can be filled out electronically and submitted by email to the Board by clicking on the Submit button in the upper right-hand corner. |

Request for Waiver of CME Requirement Waivers are governed by A.R.S. section 32-1825(C) and AAC R4-22-109. Filing a Waiver request does not exempt you from having to pay renewal or late filing of renewal fees if applicable. Please review the requirements for Waivers as listed on the form. This form can be filled out electronically but must be printed and signed before submitting by email, mail, or fax to the Board with all attachments as listed on the form. |
