2012-2013 Arizona Osteopathic Online Renewal
Please read the directions below before starting. When you are ready to begin, scroll down and click on the link to log in to the Online Renewal system.

How long will the process take? The renewal process should take between 10 and 15 minutes to complete. If you do not complete
it in one session, your data will not be saved.

Check your online profile before beginning and verify its accuracy. Review your Physician Profile before starting
the process. To view your profile return to the home page and click on "Doctor Search" - You will then be able to enter
your name or scroll down to enter your Arizona license number. If changes to your profile are required other than
address and phone number please print profile, indicate changes and fax to 480-657-7715. Attach a copy of your
post-graduate training certificate or letter of completion from the training facility associated with any change you
are requesting. Changes will not be made without this proof of completion.

Payment Method. Only Visa, MasterCard and AMEX are accepted. Please note, there is an $8.00 processing fee charged for
all online credit card transactions.

CME Audit. If your notice letter says “CME Audit = Yes”, you have been randomly selected for CME audit this renewal
cycle. You will be asked to input the CME you have completed in 2010 and 2011. Please note only CME completed AFTER
January 1, 2010 which was not used to qualify for a previous renewal is valid for this renewal cycle. Please have
your CME documents ready in order to enter the data when asked.

If you choose not to renew online you can print a blank renewal form to send with your check, money order or credit card information.
Go to For DOs > Paper License Renewal > License Renewal Form 2012-2013. Paper renewal applications are processed in the order they are received and can take up to 30 days to process and send out your wallet card.
If you know you need your wallet card by a particular date, please plan accordingly. If you use the Online Renewal, your wallet card will be sent to you within 10 days.
